To attend any event hosted by Third Eye or The Haunted Mafia, LLC, it is mandatory to complete the following form. Submitting this form online will simplify the registration process on the day of the event. Ensure that your form is submitted at least 24 hours before the event begins; otherwise, you and your party may not be included. If you have purchased tickets for multiple people, each individual must submit a form. Failing to do so for every member in your group will lead to delays during registration at the event venue. Thank you for your cooperation. Don't forget to also submit your LIABILITY WAIVER online.
